Multielectrode Pulmonary Vein Isolation Versus Single Tip Wide Area Catheter Ablation-Paroxysmal Atrial Fibrillation: Is There Any Mystery in Pulmonary Vein Isolation?

چکیده

In the past decade, catheter ablation of atrial fibrillation (AF) has evolved as one of the most frequently performed procedures in tertiary arrhythmia centers worldwide. With the observation of the actively firing trigger originating from within the pulmonary veins (PVs), ablation strategies have focused at least in paroxysmal AF patients on complete isolation of the PVs. Several different ablation devices (cryogenic energy, ultrasound, laser, etc) have been investigated to achieve more durable isolating lines around the PVs, with most of these novel ablation catheters allowing for a singleshot approach. Multielectrode-phased radiofrequency ablation probes (PVAC; Medtronic, Minneapolis, MN) have been proposed using the standard radiofrequency energy through smaller but multiple electrodes. The aim of all these ablation devices is not only to improve outcome but also to allow an easier ablation procedure eventually being performed by nonelectrophysiologists to be able to respond to the growing demand for AF ablation.

Multielectrode Pulmonary Vein Isolation Versus Single Tip Wide Area Catheter Ablation for Paroxysmal Atrial Fibrillation

Catheter ablation (CA) is an accepted treatment option for symptomatic atrial fibrillation (AF). Historically, CA has been performed with a point-by-point technique, using standard focal radiofrequency catheters and 3-dimensional (3D) navigation systems, with a procedural end point of electric pulmonary vein isolation (PVI). However, this CA approach requires advanced training and operator skil...

Single Center Experience with a Closed-Loop Irrigated Ablation Catheter for the Treatment of Human Paroxysmal and Persistent Atrial Fibrillation

Open-irrigated electrode catheters are commonly used for ablation for atrial fibrillation (AF). There are scant data available for closed-loop irrigation for ablation for AF. Proponents of open-irrigated catheters state greater patient safety from char formation, less impedance rises, and greater lesion depth. There are fundamental biophysical differences in the biophysics of the energy deliver...

A Randomized Comparison of Circular versus Single Point-By-Point Pulmonary Vein Isolation

Introduction: Point-by-point standard ablation using single-tip (conventional) catheter is the main current technique for pulmonary vein isolation (PVI) but it can be challenging. A circular, multielectrode, mapping and ablation catheter (nMARQ) may provide faster PVI. Methods: We designed a randomized comparison between circular versus conventional PVI with the primary end point of reconnectio...
